AFH Chart is currently released as a pilot prototype. It is provided "as is" and "as available". We make no warranties regarding continuous uptime or error-free data synchronization. Licensed facility operators are solely responsible for ensuring that all charting, medication logs, and records are maintained in compliance with local, state, and national regulations.

Our software enforces strict role partitions to protect data. Users agree to configure staff roles accurately (Owner, Admin, Caregiver, Viewer). Owners and Admins are solely responsible for actions performed by standard Caregivers or Viewers under their supervision, as recorded in our secure Audit Logs.
AFH Chart is a documentation tool and does not provide clinical advice or patient diagnoses. Licensed caregivers and medical practitioners must use their own professional clinical judgment when administering medications or executing care plans. AFH Chart is not liable for clinical errors, omissions in care, or data loss caused by local device failures.
We reserve the right to modify, pause, or update features within the caregiver portal or public website at any time to maintain regulatory compliance or improve service.
