At AFH Chart, we are dedicated to protecting the privacy of the caregivers, administrators, residents, and family members who use our platform. This Privacy Policy details how we collect, safeguard, and handle sensitive information in accordance with privacy laws and HIPAA (Health Insurance Portability and Accountability Act) standards.

1. Information We Collect

To provide a fully functional electronic charting system for small Adult Family Homes, we collect information within the caregiver portal under the authorization of the home's licensed operator:

• Caregiver Information: Full name, login credentials, database role (Owner, Admin, Caregiver, Viewer), and account status.
• Resident Profile Information: Name, room number, age, dietary instructions, allergy records, fall risk level, and medical code status (DNR / Full Code).
• Care Logging & Notes: Administered medications, daily ADL logs, shift handoff checkmarks, caregiver text notes, and incident report details.
• Audit Logs: Detailed timestamps tracking user operations (e.g., who recorded a medication dose, who modified a resident chart, or who updated staff profiles).

2. How We Use Your Data

All collected data is processed exclusively to deliver secure, efficient charting tools. We do not sell, rent, or share resident or caregiver information. Data usage is limited to:

• Displaying active task lists, outstanding medication passes, and shift checklists.
• Maintaining logging histories to satisfy state licensing audit requirements.
• Enforcing Role-Based Access Controls to keep resident records visible only to authorized users.

3. Data Storage and Retention

In the current portal version, all database tables are initialized, modified, and saved locally inside the user's secure browser session cache (LocalStorage). This data does not travel to any unencrypted external cloud servers unless explicit backup integrations are configured by your administrator.

4. HIPAA & Protected Health Information (PHI)

Resident records qualify as Protected Health Information (PHI) under HIPAA guidelines. We restrict access to PHI through strong technical mechanisms:

• Encryption of any local databases or cloud-sync data transfers.
• Separation of duties via staff roles. Caregivers with Viewer roles cannot make edits, and standard Caregivers are completely blocked from viewing Audit Logs or accessing Staff Management interfaces.
